Provide a cover letter or statement and (if a first time recipient) a copy of your 501c3 determination letter.

Describe who will benefit and how; how the money will be spent; total cost of the program/project and sources of other funds, especially from other Rotary clubs; and opportunities for public recognition of the Kelso Rotary Club donation.

Requests for more than $1,000 will have to provide additional, substantiating information (upon request).
